| Job Description | Performs home visits in accordance with the Nurse Family Partnership (NFP) model and guidelines, including the recruitment, retention and management of a case load of up to 25 clients. Develops therapeutic relationships with women and their families in a home visiting environment. Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains. Assists women and their families in establishing goals and outcomes. Provides education, support and referral resources in assisting women and their families in attaining their targeted goals. Consults and collaborates with other professionals involved in providing services to women and families

| Job Requirements | Bachelor’s degree in nursing and one (1) year of nursing experience in maternal/child health, community/public health, or related nursing practice. Current licensure in good standing as a registered nurse issued by the Arizona State Board of nursing. May substitute BSN with a bachelor’s degree in another discipline and five (5) years maternal/child or relevant community-based nursing experience. |
| Job Skills | PREFERRED: Home visiting experience and Bilingual English/Spanish preferred.
Knowledge of pregnancy and prenatal health. Knowledge of infant and toddler growth and development. Knowledge of health, socio-economic and cultural factors which impact maternal/child health. Knowledge of community resources. . Ability to access community resources. Good oral and written communication skills. Ability to provide family-centered care and services in a culturally competent manner. Excellent written and verbal communication skills. Basic computer skills. Motivated to utilize computer for long-distance learning. Must demonstrate the ability to maintain regular and reliable attendance. |
